Pre-Wedding Questionnaire Form
Please answer the following questions to help us understand you a little better and to create a photographic experience
according to your individual personality. This form should be submitted at least two weeks prior to your event.

I don't like posing for pictures, I prefer candid shots instead.
I want as many portrait-style pictures I can get!
I prefer to have my formal shots immediately following the
ceremony (or a different day altogether).
How much time do you have for formal portraits on your
weddingday? (1 Hour is recommended.)
I want a second photographer at the reception taking
pictures during the formal portrait session.
I like unique portraits and some photojournalism.
I want my photographer to capture funny and embarassing
moments.
I want photo documentation of my displays and guests.
During the reception, I will be providing a meal for the
photographer(s).
Forevermore Images requires a 20-minute break during the
reception. This is usually while guests are having dinner. Do
you prefer to have us take this break during another time?
Please provide a list of the events in a timeline for your
wedding day.
Please write any additional items you wish to have
photographed.
If time permits, I want pictures of me getting ready before the
ceremony.
Due to the time limit during the cocktail hour (formal portrait session), we will be limited in how many portraits we can
take. We want to respect your schedule for your special day. We will begin with immediate family portraits (parents,
grandparents, siblings, and children) then wedding party portraits, then bride and groom portraits. We wll be happy to
do more poses but ONLY if time allows. All other poses will need to be taken throughout the reception. Please fill out
this form of which people you would like to have in each pose.

If an Online Image Gallery is included with your package, Online Gallery Access Cards will be provided for you and
your guests. These cards have the password needed to enter your Online Image Gallery. Please tell us where you
would like for us to place these cards at your wedding:
1. Have the photographer to hand these cards to my guests
during the reception.
2. Place them on the Guest Book table.
3. I do not want my guests to have access to my images.
